Athletic Paperwork

Student-Athlete safety is the number priority of Trimpe Middle School. In order to ensure that all of our student-athletes are medically cleared and safe to participate in interscholastic sports, the following paperwork must be on file at Civic Memorial High School for each student-athlete.

  • Current physical, signed by a doctor, that states the student-athlete is medically cleared to participate in sports. Click here to download the IESA physical form to take to your physician. 
  • Proof of insurance. Click here to download the Proof of Insurance Form. 
    • In lieu of a student-athlete being covered by a parent/guardian's insurance, a parent/guardian can purchase insurance for their son/daughter at 1st Agency by clicking on this link. 
  • A signed Civic Memorial/Trimpe Middle school Student-Athlete Code of Conduct. Click here to download the Athletic Code of Conduct. 
  • A signed Illinois High School Association (IHSA) Concussion, Performance Enhancing Substances, and Asthma Form (one form, often referred to as the "IHSA Form" or "IHSA Concussion Form".) Click here to download this form. 

Athletic Fees

Athletic fees at Civic Memorial High School and Trimpe Middle School were changed by the Board of Education in the spring of 2017. Below are athletic fees for student-athletes and CM and TMS. As you can see, there is a graduated increase over the next three school years. 

On the below chart, "Reduced" refers to if the athlete and his/her family receive reduced price lunches as part of the Federal School Lunch Program, and "Free" refers to if the athlete and his/her family receives free lunches, as determined by the school district's food service coordinator. 

Middle School Football Athletic Fee:
The athletic fee for Trimpe Middle School football is $150/$70/$50. This increased fee is due to the costs associated with football.

Request to Play Two Sports in One Season

If a Trimpe Middle School Athlete wishes to play two sports in one season, it may be possible, provided that all coaches, the athlete, parents, and the athletic director agree and sign this form.

Once all coaches, the athlete, and parents have signed off, please turn in the form to the TMS athletic director.  

Turn In Process

You may download any of these forms here on the website and bring them to Trimpe Middle School. No student-athletes will be allowed to try-out for a team, practice for a team, or play for a team unless all of the required documents are on file in the athletic office. 

There will be an athletics table at registration starting with the 2017-18 school year and a district representative, will be on hand to collect forms that are completed and answer any questions. In addition, there will be paper copies available at Central Registration. However, if you'd like to save time, you may download the documents from the website, complete them ahead of time, and then turn them in at registration.
